Present health and social care practice-based problem (what) Identify key stakeholders in the health and social care practice-based problem (who) Contextualise the problem (national scale) Evaluate how the issue can be addressed using key principles of effective communication, values-based practice, and teamwork Reflect on how this relates to your own health and social care practice
